      What You'll Be Doing

As a Housing Social Worker with the Cornerstone Bridge Housing Program, you will provide case management services to the twenty residents living at Cornerstone. Cornerstone will serve adults experiencing chronic homelessness and living with severe and persistent mental illness and/or other disabilities.

      Essential Functions:

      • Provide intensive case management to residents including assessing for needs, identifying housing barriers, addressing mental health and/or substance use needs and supports and connecting residents with the appropriate resources
      • Develop individualized treatment plans for each resident and monitor progress towards achieving their goals
      • Assist with screening prospective residents and completing initial assessments
      • Help clients transition from the bridge housing program to permanent supportive housing
      • Facilitate or coordinate groups that are available to Cornerstone Bridge Housing residents
      • Orient new residents to the program and assists with addressing incidents as they arise
      • Maintains required data and HMIS documentation on all cases
      • Assist Housing Support Specialists with weekly room checks

The Department’s programs include Community Revitalization, Development Financing, Rental Assistance, Homeless Services, Community Outreach, Emergency Shelter, Veterans Services and Permanent Supportive Housing.

      The Basics (Required Education and Experience)

      • Bachelor's degree in Social Work or related field
      • One year of job-related experience
      • Valid Driver's License and "safe" driving record
      • Equivalent education and experience are accepted

      Beyond the Basics (Preferred Education and Experience)

      • Master’s degree in social work
      • Experience working in residential settings
      • 2 years of experience with case management services, housing, and homeless population

      How Will We Know You're 'The One'?

      • Knowledge of Housing First, Harm Reduction, Motivational Interviewing and other evidence-based best practices
      • Passion and skills for working with mental health, substance use, crisis intervention and the homeless population
      • Ability to prioritize competing responsibilities to accomplish tasks and processes accurately and timely
      • Solid rapport building skills
      • Excellent speaking, listening, and written communication skills
      • Ability to be flexible, a team player and adaptable to changing circumstances
      • Proficient skills with Microsoft Windows Operating Systems (Excel, Word and Outlook)

      Hiring Range: $23.47 - $31.68

      Emergency Service Worker Statement
