What to Expect (Job Responsibilities):
- Assist members with housing search and placement, leveraging supportive services
- Ensure proper residential authorizations are in place
- Conduct intake interviews and assess housing barriers for members experiencing homelessness
- Develop individualized goal and action plans for members promoting permanent housing
- Participate in housing collaborative events to expand resource base
What is Required (Qualifications):
- Bachelor's Degree (or equivalent combination of education and relevant experience)
- Minimum 2 years’ experience working with underserved or special needs populations
- Valid driver’s license with good driving record
- Knowledge of applicable state and federal guidelines/regulations
- Ability to travel locally 50% of the time
How to Stand Out (Preferred Qualifications):
- Bachelor's Degree in a human services or healthcare-related field
- Bilingual based on community need
- Familiarity with healthcare systems/processes, NCQA, and Interqual
- Experience working with persons who have severe and persistent mental illness
- Current Community Health Worker (CHW) Certification preferred
