      What to Expect (Job Responsibilities):
      - Assess, plan, and implement individualized care strategies for patients
      - Identify and initiate referrals for social service programs
      - Manage care plans as a single point of contact throughout the continuum of care
      - Communicate health-related information to stakeholders for quality coordinated care
      - Advocate for patients and families to ensure their needs are met by the healthcare team

      What is Required (Qualifications):
      - High School Diploma/GED (or higher) AND 5+ years of experience working with ABD/SSI population in a clinical healthcare role
      - Familiarity with Microsoft Office, including Word, Excel, and Outlook
      - Unrestricted LPN/LVN license in the State of TX with 3 years of clinical experience
      - Reliable transportation for field-based roles
      - High-speed internet access at residence

      How to Stand Out (Preferred Qualifications):
      - Ability to create, edit, save, and send documents, spreadsheets, and emails
      - Reside within a commutable distance of assigned duties
      - Experience working on Service Plans
