We have a unique opportunity for a Care Center Coordinator to join our dedicated multidisciplinary team in the Huntington’s Disease Comprehensive Center and make a significant contribution to providing specialized quality care to our patients.

The program coordinator for the HDSA Center of Excellence at Albany Medical Center will administer the grant deliverables for the HD community within the AMC catchment area, help maintain the clinic and program as outlined by the HDSA.

Essential Duties and Responsibilities include:

• Serve as the initial contact for the HD Clinic, including for Huntington Disease testing.

• Coordinate the multidisciplinary clinic between all providers for what each patient may need.

• Refer patients to other providers as needed.

• Be familiar with ongoing research studies and work with research coordinators to facilitate subject participation in trials if desired.

• Maintain a relationship with area nursing homes that have NYS funding for dedicated HD care, including coordination of monthly rounding at Ferncliff Nursing Home in Rhinebeck, NY.

• Develop a targeted outreach plan for the medical community, including other physicians, social workers, and nursing homes.

• Serve as a liaison to local support groups for PD and HDSA chapters; may include writing informational brochures and handouts regarding these support groups.

• Coordinate efforts at philanthropic giving for the HD center, including identifying and cultivating potential donors.

• Perform strategic and business planning functions in conjunction with other members of the team.

• Assist in developing the center’s budget, producing necessary financial reports

Minimum Requirements:

• Master’s degree in Social Work

• Knowledge and understanding of neurodegenerative diseases.

• Health Care Management experience.

• The ability to interact well with others and manage multiple priorities.

• Be willing to travel

• Have availability on some weekends and evenings (for community events)
