  Key Responsibilities:
  
  - Coordinate with providers and Integrated Care Nurses to support Chronic Kidney Disease (CKD) patients.
  - Serve as a single point of contact for DaVita IKC patients, coordinating resources and providing ongoing communication.
  - Perform outbound and inbound calls, completing non-clinical pathways and screenings under the guidance of the Manager.
  - Identify eligible resources for members and assist with enrollment in eligible programs.
  - Assist with the referral process and coordinate provider appointments on behalf of DaVita IKC members.
  
  Qualifications:
  
  - Intermediate proficiency in MS Word, Excel, PowerPoint, and Outlook.
  - Minimum of 2 years of experience in a professional office environment.
  - High School Diploma required.
  - Preferred experience as a Patient Care Technician or Administrative Assistant in dialysis.
  - Medical experience preferred.
