Job Description

- High school diploma or GED required, Associates degree in related field preferred.
- Minimum of 1-2 years of management experience, with increasing level of responsibility.
- Experience with geriatric population preferred.
- Experience in PACE program highly desirable.
- A genuine spirit of caring and compassion
- Valid Colorado driver’s license, adequate automobile insurance and access to reliable transportation if working in the community setting
- Ability to work collaboratively and communicate effectively
- Competitive pay structure starting at $23.00-$26.00 per hour, depending on experience and education.

What is PACE?
PACE or Program of All-inclusive Care for the Elderly is a federal and state funded program that provides comprehensive, coordinated, and proactive whole-person care to help maintain and improve seniors' health so they can live safely in their homes. There are 163 PACE programs, serving more than 72,000 participants in 32 states.
The number of people aged 55+ is increasing in Grand Junction, CO and the demand for eldercare professionals is on the rise.
The IDT & Day Center Manager will be responsible for the daily functions of the IDT and the Day Center. This includes the facilitation of the daily IDT meeting, IDT coordination of service delivery and plan of care management, and coordination of food service provision alongside dietitians. The IDT & Day Center Manager will work to ensure appropriate staffing, efficiency of operations, and care that is participant focused in the day center. This position demonstrates excellent leadership skills, maintains compliance with state, federal, and CDPHE regulations, creates and maintains effective workflow s, and promotes a delivery model of care that meets and or exceeds participant expectations and needs.
