What to Expect (Job Responsibilities):
- Conduct in-home health evaluations, including medication and medical history reviews
- Engage with patients to build trusted relationships and address health questions
- Provide diagnostic and preventive services as indicated, such as spirometry and bone density screenings
- Travel locally and within licensed states as needed for patient visits
- Conduct virtual visits when necessary
What is Required (Qualifications):
- Active, unrestricted license(s) in coverage area(s)
- Board certification as a Nurse Practitioner or Physician Assistant
- Ability to cover specified primary locations in California
- Minimum of 29.5 hours available per week
- Flexibility to travel locally and conduct virtual visits

- Experience working in home health or community health settings
