      Independent Living Coordinator – Part Time


      Classification: Non-Exempt

      Reports to: Vice President of Housing and Outreach

      JOB DESCRIPTION

      Summary/Objective

      The Independent Living Coordinator is responsible for case management services to youth and young adults who are experiencing out-of-home placement, such as foster care or residential living, homelessness, or are at-risk of homelessness in Sheridan and Johnson Counties. The Independent Living Coordinator will also assist with service coordination and case management services to individuals and/or families who are experiencing homelessness or at risk of homelessness in Sheridan & Johnson County.

      Essential Functions/Competencies

      • Provide supportive services related to youth and young adults who have been in the foster care system, juvenile justice system, are homeless, or are at-risk of homelessness
      • Provide supportive services related to youth and young adults who have been in the foster care system, juvenile justice system, are homeless, or are at-risk of homelessness
      • Conduct intake paperwork for new participants to establish program eligibility
      • Provide case management with participants that include, but are not limited to, education, housing stability, budgeting, financial wellness, and employment
      • Conduct outreach and awareness through education and collaboration with other community service providers and human service agencies
      • Provide appropriate referrals for clients based on their case plan needs
      • Record and maintain case management data and document all services provided by the Independent Living Program
      • Provide life skills classes and learning opportunities for youth in Sheridan and Johnson counties
      • Assist with Cheyenne Housing Inspections as needed
      • Assist with Homeless Outreach Intakes
      • Assist with Homeless Outreach by responding to inquiries through phone calls and email.
      • Other duties as assigned

      Supervisory Responsibility

      This position has no direct supervisory responsibilities.

      Work Environment

      This job operates in a professional office environment. This role routinely uses standard office equipment such as laptop computers, photocopiers, and smartphones.

      Physical Demands

      While performing the duties of this job, the employee is regularly required to talk or hear. Specific vision abilities required by this job include close vision and ability to adjust focus. This would require the ability to lift files, open filing cabinets, and bend or stoop as necessary.

      Position Type/Expected Hours of Work

      This is a part-time position.

      Travel

      Travel would include going to Johnson County to assist clients and traveling in town to assist clients.

      Required Education, Experience or Eligibility Qualifications

      • Associate degree with a minimum of two years of case management or related experience

      -Or-

      • High School diploma or equivalent and a minimum of four years of case management experience or related field experience.
      • Must have a valid driver's license
      • Must complete a Federal Criminal Background Check with an approved status.

      Preferred Education and Experience

      A Bachelor's degree in human services, behavioral science, or a related field with one year of case management experience.
