Job Description

- Support surveillance activities within the LTC Hub's associated long term care and retirement homes and congregate care settings to facilitate preventing and controlling outbreaks of infection;
- Support outbreak management;
- Act as a resource for clients, providing support services including occasional evening and weekend calls;
- Provide education and training to care providers and other staff members;
- Perform continuous Quality Improvement responsibilities;
- Participate in professional development and education activities/programs to ensure excellence in the provision of Infection Prevention and Control services within the LTC Hub;
- Perform cross-functional and/or other duties consistent with the job classification, as required.
Qualifications
- Completion of a Bachelor’s Degree program, required; Bachelor of Science in Health or allied Health Sciences, or recognized equivalent, preferred.
- Minimum four (4) years of relevant Infection Control experience.
- Demonstrated teaching experience, required.
- Infection Prevention and Control Canada(IPAC-Canada) membership, required.
- Experience working in a Hospital, Long-term Care Facility or public health facility.
- Current license or registration from appropriate bodies dependent on training (i.e. College of Medical Laboratory Technologists, College of Nurses of Ontario, etc.).
- Excellent customer service skills.
- Excellent time management skills/
- Excellent interpersonal and communication skills; ability to establish and maintain an effective working relationship with others.
- Demonstrated ability to exercise initiative and good judgement with effective organizational skills and ability to multi-task.
- Excellent problem solving and conflict resolution skills.
- Computer skills and familiarity with multiple software programs.
- Ability to work independently with minimal supervision.
- Knowledge of applicable legislative, UHN and/or departmental policies.
- Compliance with confidentiality requirements.
- Experience in designing and conducting research with advanced statistics capabilities, preferred.
- Certification from the Certification Board of Infection Control (CBIC), preferred.

Position Summary
The Infection Control Practitioner I, LTC, works within the Infection Prevention and Control department's Long Term Care (LTC) Hub. The IPAC LTC Hub provides support and education to assigned LTC homes, retirement homes and non-LTC congregate care settings. In this role, the Infection Control Practitioner I works in the capacity of support to meet the needs of an evolving infection prevention and control practice.
