      Description

      SUMMARY :

      Provide assessment, intake, and screening services in a MAT milieu for prospective patients. Point person for all new individuals or facilities seeking MAT services. Deliver recovery based clinical services in individual, family and group venues for new intakes during the orientation phase of treatment. Maintain an active caseload of patients transitioning to the intensive outpatient level of care. Complete all required corporate and CBH reports, oversee and approve transfer packets, oversee and assist intake staff with completing all Biopsychosocials. Maintain patient census and clinical charts in accordance with NET Steps, Pennsylvania Department of Health standards, and other city, county, federal and JCAHO/CARF standards, and pursuant to payer contracts and requirements. Supervise and provide direction to intake coordinators.

      Qualifications


      MINIMUM QUALIFICATIONS :

      A Bachelors’ Degree from an accredited college with a major in medicine, chemical dependency, psychology, social work, counseling, nursing (with a clinical specialty in the human services) or other related field and 2 years of clinical experience in a health or human service agency which includes 1 year of working directly with the chemically dependent conducting screenings, intakes, and assessments.
