      Summary, Job Description, and Qualifications


      Job Title: Intake Specialist

      Reports To: Central Intake Supervisor

      Summary:

      As a partner in building brighter futures for youth and their families, the Foster Care Intake Coordinator will be responsible for the facilitation of intake referrals for foster care programs across the agency as it aligns with The Village Network’s Mission, Vision & Values.

      Essential Tasks, Duties, and Responsibilities:

      • Review and assess initial intake information from a therapeutic perspective to determine course of action and potential match to network, foster parent, or residential programs
      • Maintaining effective communication with each referral source until the child is placed with TVN. Serving as a liaison between referring agencies, TVN network offices, directors and foster parents
      • Document and track all referrals and outcomes in identified agency database
      • Attend referral meetings with county agencies, probation or other referral agencies as necessary
      • Establish and maintain a positive working relationship with all levels of TVN staff by participating in joint meetings, and problem identification and resolution.
      • Establish and maintain a supportive and positive working relationship with foster parents, referring agencies, and other TVN stake holders
      • May be on call 24 hours or on a rotating basis to address after hours and Holiday referrals
      • Must be able to attend new hire orientation and be available to be in-person at designated organization events
      • Performs other duties as required.

      Knowledge, Skills, and Abilities:

      • Minimum of a Bachelor’s Degree; Social Work, Human Services, or related field desired.
      • Valid Ohio Driver’s License and maintains a driving record that allows that individual to be insurable with the insurance company providing The Village Network with vehicle insurance.
      • Demonstrates excellent verbal and written communication skills, exceptional customer relation skills, ability to motivate others, and the ability to solve problems through creative solutions.
      • Attention to detail and accuracy and ability to multitask
      • Possesses excellent knowledge of best practices in the field of foster care, trauma informed care and child welfare.
      • Ability to use a computer; software, agency databases and online systems and other Microsoft Office products.

      Physical Demands:

      • Ability to comply with The Village Network requirements for using restraints including sufficient strength and completion of extensive training.
      • Ability to use a computer; proficiency in Word and Electronic Health Record (EHR).
      • Ability to handle bending, stooping, lifting, pushing, reaching, and walking for periods of time.
      • Must be able to lift 20 pounds independently.
      • Ability to communicate (verbal and written) with all levels or personnel, internal and external to the company.
