Our home infusion pharmacies provide infusion therapy solutions for patients in the comfort of their home. We provide a range of therapies that include, but are not limited to, antibiotics, inotropes, IVIG, and TPN. The Intake Specialist I will be responsible for providing patient-centric solutions that improve access, affordability, and adherence to enhance patients' lives. The Intake Specialist I will be responsible for entering patient demographic information into the computer database according to company policy and assisting in setting up all home services for new referrals.

Responsibilities:
-Act in accordance with Paragon Healthcare values, including maintaining professional ethics, boundaries, and client confidentiality and working from a strength-based perspective using evidence-based practices.
-Maintain accurate documentation and record-keeping systems, including but not limited to referral database, referral paperwork, and utilizing electronic health records.
-Travel to meet with clients and conduct intake procedures.
-Collaborate with Admissions RPh to ensure that patients are clinically appropriate for Paragon Healthcare and make decisions about pairing patients to what would best support their therapy.
-Work closely with the HIT/Sales team to coordinate admissions and ensure consistent communication.
-Provide excellent customer service and communicate effectively to referring agencies and individuals.
-Regularly engage in outreach and marketing activities to provide potential referral resources within the policies, procedures and mission of Paragon Healthcare and maintain database of referring agencies and organizations, and community resources.
-Contact floor nurse and patient within 2 hours from benefits to determine discharge plans and make patient contact. Follow up on discharge plans daily without being prompted by supervisor or pharmacist, or AE.
-Call the following MD to confirm that MD will follow the patient’s therapy post-discharge on all patients.
-Ensures that each patient chart is complete, including ER contact, AE and modifier, medication profile, access, height/weight, allergies, and diagnosis code before discharge and documents are appropriately separated and assigned.
-Post complete template (including following MD, access, HH visit date and time, and appropriate payment info) and complete chart in CPR, including medications in Medispan, within 4 hours from benefits for same-day discharges.
-Completes chart audits in the employee portal weekly without a prompt from a supervisor.
-Calculates and quotes OOP costs and completes assessments in CPR.
-Assists in the pharmacy when needed.
-Can perform technician responsibilities outlined in IS Tier I and Tier II in addition to dispensing and compounding as appropriate.
-Perform any other duty or task that your supervisor or their designee may request from time to time.
About You:
-High school diploma or equivalent (GED).
-Certified Pharmacy Technician preferred.
-Preferred continuing education necessary to maintain current licensure in all states licensed.
-Bilingual preferred.
-Health Status Requirements:
-Ability to lift 20 pounds.
-Appropriate immunizations current.
-TB screen (as required by policy).
-Hepatitis B vaccine or signed waiver.
-Other state required health tests allowable by law.
