      Job summary

      The Integrated Discharge Team at Somerset NHS Foundation Trust focuses on delivering efficient, sustainable, and patient-centred discharge processes. Our goal is to ensure patients are discharged from acute hospitals with the necessary support to promote independent living or alternative care when needed. The team integrates multiple services, including the Transfer of Care Hub and Discharge Lounge, working across acute sites in Somerset to improve patient flow and outcomes.

      We value our team members highly so development and training within this role will be offered and prioritised.

      There is an expectation that hours are worked flexibly over a rota basis which includes day shifts and weekends.

      Main duties of the job

      As an Integrated Discharge Nurse, you will play a vital role in the daily operations of the Discharge Team, supporting the Senior Team in managing clinical and non-clinical aspects of the service. You will coordinate patient discharges, provide expert guidance, and ensure effective communication with internal and external stakeholders. Your primary focus will be to improve patient flow, reduce length of stay, and enhance patient experience by supporting seamless discharge processes.
