      Sun River Health provides the highest quality of comprehensive primary, preventative and behavioral health services to all who see it, regardless of insurance status and ability to pay, especially for the underserved and vulnerable. Sun River Health is a Federally Qualified, Non-Profit Health Center serving communities in Suffolk, Rockland, Orange, Dutchess, Ulster, Sullivan, Columbia and Westchester County. Sun River Health is currently seeking a full-time Integrated Support Care Manager to join our team in Wyandanch.

      Responsibilities:

      • Completes Care Management Intakes/assessments
      • Develops and monitors patient care plans
      • Develops linkages and refers patients for additional support services
      • Completes patient Recovery Support and or Crisis plans
      • Communicates and works collaboratively with the patient and care team
      • Provides Naloxone training to staff and patients
      • Conducts/supports HIV testing and links patients to appropriate services and follow up
      • Participates in case conferencing with care teams, inclusive of providers
      • Maintains current case records and other documentation as required and prepares necessary reports on time
      • Demonstrates knowledge of stages of change, substance use disorders, HIV, HCV, and other co-occurring conditions
      • Completes on-going training to enhance skills and patient care
      • Empowers, educates, and links patients to HIV, HCV, STI testing/treatment, PrEP, substance use, and other primary care services
      • Conducts quality improvement projects and other duties as assigned

      Requirements:

      • HS Diploma
      • Associates Degree Preferred
      • CASAC License Preferred
