- Monitor due dates and the need for upcoming assessments.
- Sc hedule and conduct assessments of individual support needs according to specified timelines.
- Conduct information gathering interviews with a diverse population.
- Review client records to gather information relevant to the assessment.
- Complete progress notes to reflect completed services.
- Different types of disabilities and the medical, physical, and psychological implications they create.
- Intellectual and Developmental Disabilities System and the difference between all service elements.
- Federal, state, and county laws, codes, rules, and regulations governing public health.
- Human relations, interviewing, counseling, and writing skills.
- Department of Health Services computer program and other health services software.
- Client assessment, service planning, and paraprofessional support.
- Customer service and community outreach.
- Engage with individuals with intellectual and developmental disabilities to assess their support needs.
- Effectively managing and prioritizing tasks to meet deadlines and workload demands.
- Preparing and completing assessments that meet all requirements.
- Managing and assuring compliance to information management and client confidentiality requirements.
- Following directions and interacting professionally.
- Working within a team and collaborating with multiple team members.
- Establishing and maintaining effective working relationships with staff, professional agencies, and community members.
- High school diploma or GED equivalent;
- AND three (3) years of human services related experience;
- OR any equivalent combination of training, education, and experience that provides the required skills and knowledge to perform the essential functions of the job.
- Experience conducting assessments in the Oregon I/DD service delivery system.
- Knowledge of case management in the field of social services.
- Experience and/or knowledge of Positive Behavior Support and Person Centered Thinking.
- Knowledge of Oregon Intellectual and Developmental Disability (I/DD) case management practices.
- Knowledge of Oregon Administrative Rules pertinent to the provision of services to those with Intellectual/Developmental Disability services.
- Bilingual in English and Spanish language.

Deschutes County Health Services supports over 400 staff in Behavioral and Public Health programs working to promote and protect the health and safety of the community in a broad variety of community based settings.Deschutes County Intellectual and Developmental Disabilities Program is the Community Developmental Disabilities Program (CDDP) providing case management and other IDD related services for Deschutes County. ABOUT THE JOB:
We are seeking a dedicated and detail-oriented professional to complete functional needs assessments, with a primary focus on the Oregon Needs Assessment (ONA), for individuals with intellectual and developmental disabilities (I/DD). This role supports individuals across a variety of settings, helping ensure they receive services that are tailored to their unique needs. Must be willing to work a flexible schedule when necessary, to meet client and department needs.
This position will be primarily housed at a Bend or Redmond office location, but is expected to travel throughout the Deschutes County area to complete interview assessments in various locations.

Preferred Qualifications:Possession of or ability to obtain a valid Oregon driver's license within 30 days of hire date. The employment offer will be contingent upon presentation of an acceptable and verifiable driver's license, pre-employment screening for criminal history, driving history, and controlled substances (NOTE: Positive test results for marijuana use may result in rescission of a contingent offer of employment). This screening must be completed with satisfactory findings in order for a formal offer of employment to be extended.
Must have dependable transportation and be willing and able to drive throughout the State of Oregon in sometimes adverse weather conditions.
Position will require some travel throughout the Central Oregon area to meet client needs, and to other locations within the state (including Portland, Salem and Eugene) for training and development purposes.About the company
