- Act as a resource for inmates, offering guidance and support to help them navigate the correctional system effectively.
- Conduct initial assessments to identify incarcerated individuals who are homeless or at risk of homelessness.
- Advocate for inmates' rights and ensure they know available programs and services promoting access to supportive services and alternatives to incarceration.
- Provide education and training on homelessness, reentry, and effective service delivery to jail staff and community partners.
- Provide guidance and resources to address immediate needs, including shelter, food, clothing, and hygiene.
- Collaborate with local homeless shelters, transitional housing programs, and community organizations to secure housing options for individuals upon release.
- Connect individuals with healthcare services, mental health resources, substance abuse treatment, and other support programs.
- Foster a culturally sensitive environment by addressing inmates' unique needs and concerns from diverse backgrounds.
- Assist in promoting cross-cultural understanding and communication within the facility.
- Collaborate with Jail staff, social workers, counselors, and other relevant personnel to facilitate effective communication and provide culturally competent services.
- Attend meetings and training sessions to stay updated on policies and procedures.
- Maintain accurate records of inmate interactions and services provided, ensuring confidentiality and compliance with facility regulations.
- Mediate disputes or conflicts that may arise due to language or cultural differences, working to de-escalate tense situations.
- Assist in educating correctional staff on cultural sensitivity and effective communication strategies.
- Performs other assignments and duties as required and/or necessary
- A bachelor's degree in social work, psychology, criminal justice, counseling, or a related field is often required.
- Five (5) years of progressive experience working in case management, social work, or a related field.
- An equivalent combination of education, experience, and preferred qualifications.
- Knowledge of community resources for housing, employment, healthcare, and social services.
- Cultural sensitivity and an understanding of the challenges faced by incarcerated individuals.
- Understanding of legal issues related to reentry, such as parole and probation requirements, housing, employment barriers, and access to social services.
- Empathy for clients' experiences and patience when working through complex issues are essential traits for a reentry case manager.
- Commitment to social justice, equity, and the dignity of all individuals.
- Commitment to maintaining confidentiality and adhering to ethical standards in client interactions and case management.
- Proficiency in using computer software for data entry, case management systems, and generating reports.
- Strong organizational skills are needed to maintain case files, document client progress, and meet reporting requirements.
- Skills in counseling and conflict resolution are valuable for helping clients address emotional and psychological challenges.
- Excellent verbal and written communication skills are crucial for effective client interaction, documentation, and report writing.
- The ability to listen actively and empathetically to clients' needs and concerns is vital.
- Proficiency in conducting comprehensive assessments of client needs and creating individualized reentry plans is fundamental.
- The ability to set realistic goals and monitor progress is essential for success.
- The ability to collaborate with other professionals, such as parole officers, mental health specialists, and substance abuse counselors, is important for holistic client support.
- A Master’s degree in social work, psychology, criminal justice, counseling, or a related field.
- Experience working with individuals experiencing homelessness, incarceration, or other vulnerable populations.
- Experience with reentry or rehabilitation programs, including external resources and referral process.
- Familiarity with the criminal justice system and the challenges faced by formerly incarcerated individuals
- 40 hours per week
- Possibly including Weekends and Holidays

Under the general direction of the Jail-Based Homeless Population Liaison will serve as a crucial link between the incarcerated homeless population and community resources aimed at addressing homelessness. This role involves facilitating communication, providing support services, and coordinating reentry assistance to ensure a smooth transition for individuals leaving incarceration and facing homelessness.

Duties and Responsibilities:
