2024-116 POSITION: Judith P. Hoyer Early Care & Education Family Service Coordinator DEPARTMENT: Elementary Office REPORTS TO: Principal/Supervisor SALARY: Grant Funded LOCATION: Westmar Early Learning Center Description The Family Service Coordinator, a grant funded position, implements and maintains family-based case management for families at the Judy Center. All work should be family-centered and strength-based, directed at establishing trusting relationships to support home environments and family systems. Work Schedule & Supervision This is a full-time, permanent position. Workdays will follow the ACPS teacher and staff calendar with an additional 18 per diem days in the summer. The position will require applicants to intermittently work beyond the normal workday (at an hourly rate), and occasionally travel for out of county business. The Family Service Coordinator is supervised by a school administrator and the Early Learning Supervisor at the district-level. Minimum Qualifications An earned Bachelor’s degree in psychology, social work, human development, education, or related field Minimum 3 years of experience in case management, service coordination, early childhood environments, or related field Demonstrated ability and proficiency in technology use Excellent oral and written communication skills Ability to work collaboratively and / or independently based on the task Highly motivated to support families through home visiting and referrals to outside agencies for wraparound services. Knowledge of family resources within the public school system and throughout the community Ability to assess family needs and develop SMART goals Responsibilities and Expectations Case Management Coordinate referral of comprehensive services for young children aged birth through five, and their families based on their expressed needs Work one-on-one with families to develop case management goals, conduct needs assessments, schedule follow-up communication, and monitor progress, providing ongoing support to completion Maintain family confidentiality Maintain (documentation and data for all case management services rendered evaluation purposes Participate in recruitment, enrollment, and maintaining data on families who enroll in the program. Collaborate and partner with agencies providing services for young children and their families to include planning and co-facilitation Maintain ongoing, clear communication with the site-based coordinator Participate in IFSP/IEP meetings and school-based PST meetings Attend and participate in collaborative meetings with partnering agencies, school-based committees, and required Judy Center regional and annual meetings. Attend the mandatory Judy Center conference held once per year in Cambridge, MD. Outreach Develop strategies for increasing program visibility and expansions, including community outreach, recruiting families and new family service partners. Assist with development and implementation of parent-child activities / playgroups that align with readiness expectations Work Judy Center and partner outreach events that include evening hours (twice monthly) and occasional weekend programs, including but not limited to the School Readiness Fair and the Allegany County Early Childhood Festival.
