      Position Summary:

      The Licensed Practical Nurse (LPN) provides direct nursing care to residents in a long-term care setting under the supervision of a Registered Nurse or physician. The LPN is responsible for promoting residents' health, safety, and well-being through the delivery of compassionate and skilled care in accordance with individualized care plans and regulatory standards.

      Key Responsibilities:

      • Provide direct nursing care including monitoring vital signs, administering medications, and assisting with activities of daily living (ADLs).

      • Observe residents and report changes in condition or behavior to the RN or physician.

      • Perform basic medical procedures such as wound care, catheter care, and treatments as ordered.

      • Document nursing care and resident status accurately and in a timely manner.

      • Assist in the development and revision of care plans.

      • Supervise and provide direction to Certified Nursing Assistants (CNAs).

      • Support residents' rights and maintain a respectful, dignified environment.

      • Communicate effectively with residents, families, and members of the healthcare team.

      • Comply with infection control, safety, and regulatory guidelines.

      • Participate in shift reports, training sessions, and quality improvement activities.

      Qualifications:

      • Current and valid Practical Nurse license (LPN) in [State Name].

      • Graduate of an accredited LPN program.

      • Current CPR/BLS certification required.

      • [6 months – 1 year] experience in long-term care or geriatrics preferred (new grads may be considered).

      • Strong observation, documentation, and communication skills.

      • Compassionate, patient-focused, and team-oriented.

      Working Conditions:

      • Physical requirements include lifting, bending, walking, and standing for long periods.

      • May require working evenings, nights, weekends, and holidays.

      • Potential exposure to infectious diseases and bodily fluids.
