- Accurately collect data needed for the appropriate portion of the integrated assessment.
- Take and record vital signs properly.
- Observe for, and document changes in patients’ appearance, behavior and ability to work toward treatment plan goals.
- Communicate changes in patient’s condition to the Charge Nurse.
- Provide structure and maintains a therapeutic environment in collaboration with the patient and other health care providers.
- Maintain awareness of patients’ individualized plan of care including goals and interventions.
- Provide information to the Charge Nurse that assists in the development and revision of the treatment plan.
- Document the treatment plan, nursing interventions and the patient’s response to the intervention.
- Education & Licensure (if applicable): Graduate from an accredited professional vocational school of nursing or vocational training as a Licensed Practical Nurse. Licensed to practice vocational/practical nursing from the State Board of Nursing.
- Experience: Minimum one (1) year as a LPN in psychiatric nursing preferred, with six (6) months experience in the specialized area of psychiatry also preferred but not required.
- Additional Skill Requirements: Must pass a medication administration exam before administering medications. CPR certification and successful completion of Crisis Prevention and Intervention (CPI) including restraint and seclusion policies, within 30 days of employment, and prior to assisting in restraining procedures.

Compensation:
