- The Licensed Practical Nurse delivers nursing care to assigned patients in collaboration with the Staff Registered Nurse/Advanced Registered Nurse, and is accountable for patient care administered.

- Core Job Functions
- Comply with ethical, legal, and regulatory frameworks of nursing and scope of practice as outlined in the LPN practice act. Function as an advocate for the patient, maintaining confidentiality.
- Utilize interpersonal and therapeutic communication skills when interacting with patients, families, and members of the Provider and RN. Communicates relevant, accurate, and complete information.
- Assess data related to basic, physical, developmental, spiritual, cultural, functional, and psychosocial needs of the patient. Collects data within established protocols and guidelines from various sources. Documents data collection, assessment, and communicates findings to appropriate healthcare members.
- Contributes to formulation of nursing careplan for patients in consultation with the RN and in collaboration with patient, families, healthcare members using established nursing diagnoses and nursing protocols. Prioritize nursing care needs of clients. Assist in the review and revision of nursing care plans with the RN to meet the changing needs of the patient.
- Identify and honor the emotional, cultural, religious, and spiritual influences on the patients health. Provide competent, safe, therapeutic, and individualized nursing care to patient. Utilize teach back with patient and families regarding individualized health needs in keeping with the LPN knowledge, competence, and scope of practice.
- Assists in the coordination and implementation of an individualized plan of care for patients and families. Direct aspects of patient care to qualified unlicensed assistive personnel commensurate with abilities and level of preparation and consistent with legal and regulatory framework
- Administers medication utilizing the 5 rights and available safety equipment and processes. Utilizes clinical judgment with patient assessment and prescribed therapies to ensure promotion of goal obtainment. As a leader holds healthcare providers accountable for the standard of care.
