Key Details:
- HomeCare HomeBase (HCHB) certification required – no exceptions
- Must have active RN license in hand
- Hospice experience required
- Productivity expectation: 24–30 visits/week
- Overtime must be pre-approved in writing by Administrator
- Service coverage radius: Up to 60 miles from the facility address
- 2-week cancellation notice required
