  • Monday - Friday, 8am - 5pm schedule
  • 12 - 15 patients per day
  • Hybrid role -- 3 days field work, 2 days telehealth
  • Primary care clinic and home health setting
  • Focus on geriatric patient population
  • Independent practice with home visits required
  • Telehealth experience needed
  • 3 years adult primary care experience required
  • We provide complimentary housing and travel
