- Serve as the primary liaison among patients, CIS physicians, nurse practitioners, clinical staff, and hospital partners to support high-quality outpatient care for chronic cardiovascular patients.
- Coordinate and manage all Cardio@Home service lines, including remote patient monitoring devices, dashboards, and new initiatives as they develop.
- Monitor web-based dashboards to track patient status and ensure continuity of care across multiple departments and service lines.
- Perform timely reviews of medical records, analyze care delivery, and ensure adherence to evidence-based protocols for CHF and AMI management.
- Maintain accurate and comprehensive databases to support reporting, analysis, and quality improvement.
- Complete concurrent chart reviews and collaborate with clinical staff to ensure timely, appropriate care that meets program criteria.
- Identify deviations from medical guidelines and escalate concerns to supervision.
- Provide education, training, and support to CIS staff regarding Cardio@Home programs.
- Assist in presenting quarterly data, contributing to quality improvement and strategic planning.
- Participate in community outreach, education initiatives, and marketing efforts related to Cardio@Home.
- Support business development efforts to expand remote patient monitoring and virtual care programs.
- Perform virtual and telephonic outreach, including scheduling clinic visits to ensure treatment compliance and appropriate follow-up.
- You are clinically strong and skilled at assessing and coordinating care for cardiovascular patients.
- You have excellent organizational skills and thrive in a role that requires multitasking and prioritizing.
- You communicate clearly and professionally with patients, clinicians, and hospital partners.
- You’re comfortable working with remote monitoring tools, dashboards, and electronic medical records.
- You enjoy educating others and supporting the growth of innovative care programs.
- You are motivated by improving patient outcomes and reducing preventable hospital readmissions.
- Strong clinical knowledge and analytical skills to plan, evaluate, and monitor patient care.
- Excellent verbal and written communication abilities.
- Highly organized, self-motivated, and able to prioritize effectively.
- Proficient in electronic medical records and related computer applications.
- High school diploma required
- Licensed Practical Nurse (LPN) license in the state of Louisiana
- Basic Life Support (BLS) certification required; ACLS optional
- Minimum 2 years of experience as an LPN, or 1 year of experience as a cardiology LPN
- Ability to work a flexible schedule; primarily daytime hours with occasional evenings or weekends

About the Role
The LPN Cardio@Home plays a crucial role in supporting patients with chronic cardiovascular conditions, including Congestive Heart Failure (CHF) and Acute Myocardial Infarction (AMI). Serving as the central point of coordination for all Cardio@Home initiatives, this role ensures seamless communication between patients, physicians, hospital partners, and CIS care teams. The coordinator supports remote patient monitoring, care management, patient education, quality assessments, and clinical outcomes—ultimately helping reduce readmissions and improve long-term heart health.
