What to Expect (Job Responsibilities):
- Assess and re-evaluate patient needs using electronic health records and other tools
- Create and manage Integrated Care Plans in collaboration with patients and caregivers
- Advocate for patients and assist them in navigating the health system
- Follow up with patients to ensure needs are met and monitor for compliance
- Document care coordination activities in a timely and clear manner
What is Required (Qualifications):
- Alabama Licensed Practical Nurse (LPN) in good standing
- Minimum of three years of experience in nursing
- Experience working with and accessing community resources
- Proficiency in accurate and timely documentation in electronic health records
- Ability to work effectively within multi-disciplinary teams
How to Stand Out (Preferred Qualifications):
- Experience working with special populations as required by CCBHC
- Demonstrated ability to manage multiple priorities effectively
