What to Expect (Job Responsibilities):
- Identify and coordinate care for patients in the Remote Patient Monitoring (RPM) program and those discharged from the hospital through the Transitional Care Management (TCM) program
- Collaborate with patients and families to assess care needs and provide education on chronic and complex health conditions
- Work closely with the Mobile Integrated Health (MIH) team and primary/specialty care teams to evaluate patient progression
- Facilitate follow-up care and ensure optimal health outcomes for patients
- Maintain accurate documentation and utilize EMR systems for charting and reporting
What is Required (Qualifications):
- Licensed Practical Nurse (LPN) certification
- Minimum of three (3) years of experience in clinical or community resource settings, care coordination, or case management
- Completion of two years of college coursework; Bachelor's degree preferred
- Basic Life Support (BLS) Certification
- Local knowledge of community healthcare and social resources
How to Stand Out (Preferred Qualifications):
- Ability to speak a second language, preferably Spanish
- Experience with EMR and computerized charting, particularly with Epic systems
