What is Required (Qualifications):
- WI unrestricted Licensed Practical Nursing license
- 3+ years of experience in community health or managed care setting
- Experience in long-term care, home health, hospice, public health, or assisted living
- Proficiency in MS Word, Excel, and Outlook

How to Stand Out (Preferred Qualifications):
- Experience in managing populations with complex medical or behavioral needs
- Experience in a Wisconsin Managed Care organization
- Experience working with patients/members in community and home-based settings
- Advanced computer skills including EHR documentation, MS Office, etc.
