  BRIEF SUMMARY OF THE POSITION

  The primary responsibility of this position is to represent the organization in leading our community to improved health through community education and health screening. The Licensed Practical Nurse (LPN) will perform assigned duties in accordance with established policy and procedures. The LPN will work cooperatively in maintaining standards of nursing care within the framework of the Ohio Nurse Practice Act. In doing so demonstrates, creates, and fosters a work environment that is consistent with the Mission, Vision, and Values of the Aultman Health Foundation and provides care that consistently complies with the Aultman Corporate Service Standards and high reliability culture.

  • Maintains Aultman standards of confidentiality, including the HIPAA Privacy Rule.
  • Consistently maintains a professional appearance.
  • Demonstrates flexibility in meeting community needs and responds positively to changes in workflow.
  • Identifies and addresses needs of the community members including psychological, social, cultural, ethnic, and religious/spiritual.
  • Conducts community health screening and educates community members on results and risk factors and makes appropriate referrals.
  • Formulates and implements a community education plan based on identified learning needs.
  • Interacts with and contributes to the professional development of peers and colleagues.
  • Consistently communicates requests, suggestions, concerns or problems to appropriate personnel.
  • Basic computer and software skills.
  • Other duties as assigned.

  EDUCATIONAL/EXPERIENCE REQUIREMENTS

  Minimum Education/Experience

  • Graduate of state board approved nursing program.
  • Currently Licensed Practical Nurse (LPN) in the state of Ohio
  • Valid driver’s license
  • Basic Life Support certification

  • Varied: days, afternoons, weekends
  • Sitting/standing/moving about during working hours
  • Hazardous Exposure Category – I – tasks that involve exposure to blood, bodily fluids, or tissues

  PHYSICAL REQUIREMENTS

  O = Occasionally = 0-33% of the work shift or 0-32 repetitions.

  F = Frequently = 34-66% of the work shift or 32-200+ repetitions.

  C = Constantly = 67-100% of the work shift or 200+ repetitions.
