      What to Expect (Job Responsibilities):
      - Assist with pre-visit planning for Transitions of Care visits to ensure readiness and care gap closure
      - Provide patient education on health preservation, disease prevention, and self-management interventions
      - Manage patient communications including phone calls, triage, and medication refills
      - Coordinate referrals and the acquisition of durable medical equipment (DME)
      - Maintain accurate documentation of patient interactions in the electronic medical record (EMR)

      What is Required (Qualifications):
      - Graduate of an accredited Licensed Practical Nurse program
      - High School Diploma, GED, or equivalent formal education
      - Minimum of one (1) year of clinical nursing experience
      - Current active Arkansas Licensed Practical Nurse license
      - Strong organizational and communication skills

      How to Stand Out (Preferred Qualifications):
      - Associate's Degree in Nursing
      - Minimum of two (2) years of experience in an ambulatory clinical environment with direct patient care
      - Current CPR certification
      - Experience with EPIC electronic health records
