- This is a DRIVING position seeing patients within the Dayton/Montgomery County and Springfield/Clark+Champaign County areas of coverage
- This is FT day shift, 8am-8pm with an every other weekend rotation
- In this role you will be 1:1 with the patient at the bedside-these patients are actively declining
- We provide superior care and superior services to patients at their end of life journey. Only those who have a heart for hospice will succeed
- Registered LPN with the State of Ohio without any board actions.
- One year of nursing experience preferred.
- CPR Certified.
- Computer skills sufficient to properly document services and care.
- Ability to drive during daytime, nighttime or inclement weather.
- Valid Driver's License with Safe Driving Record.
- State Minimum Automobile Insurance Coverage.

The Crisis Care LPN will provide quality nursing care to patients in a manner that meets the patient and family needs, working under the direction and supervision of an RN.
