Job Description
- Req#: REF2607P
- Document interdisciplinary progress notes that provide a comprehensive reflection of participant care.
- Administer medications and treatments in strict adherence to physician orders. Maintain accurate and up-to-date drug and narcotic records for each shift.
- Report any discrepancies to the center manager and follow proper procedures for drug and narcotic disposal.
- Collaborate with the care plan team to assess and address residents' physical, social, nutritional, spiritual, and activity-related needs.
- Perform home visits with participants as needed.
- Coordinate home care schedule.
- One year of experience working with a frail or elderly population.
- Active Pennsylvania Licensed Practical Nurse (LPN) License.
- Current CPR and First Aid certification.
- Valid Pennsylvania Driver’s License.
- Compassion, patience, and genuine desire to make a positive impact on the lives of seniors.
