      SUMMARY OF JOB

      Working under the direction of supervisor and/or Nurse Manager, directs and takes responsibility for total care of residents on a team. Responsible for implementing interventions designed to achieve the goals and objectives of the resident plan of care.

      CONNECTING TO MISSION:

      All work is performed within a resident centered frame work in collaboration with other healthcare professionals, residents and their families. Functions as a role model for staff. Recognizes the value of each person as an individual. Supports and Promotes Resident/Patient strengths and abilities, capacity for social contribution, unique values, preferences, living habits and autonomy of choice. Embraces a vision of aging that is meaningful and relationship based.


      RECRUITMENT REQUIREMENTS

      Current LPN license

      Previous experience preferred

      Demonstrates position communication skills

      Ability to relate to staff in a professional manner

      Must be flexible with scheduling

      Computer experience preferred

      Demonstrates sensitivity to residents emotional, social, mental health and spiritual needs

      Previous experience in a rehab setting preferred.

      Must be able to interact well with physicians and other health care professionals

      Able to exercise sound professional judgment

      Able to respond to emergency
