Now a part of the As apart of the VNA Health Group, this team of primary care clinicians continues to grow.

      Visiting Physician Services specializes in treating adult and geriatric homebound patients. We make house calls to private residences, senior housing buildings and assisted living facilities in Ocean, Monmouth, Middlesex, Somerset, Union, Essex, Passaic, and Bergen Counties.Our practice is comprised of physicians, nurse practitioners and physician assistants with the mission of treating elderly, frail, homebound patients with dignity and compassion .We are currently looking for an LPN to join our team, this is a hybrid position remote (via telephone) & making emergent visits to sick, homebound patients in Middlesex and Ocean County, as needed. While working with a team of clinicians to provide responsive & appropriate care for patients. Tasks include; assessing patients, recording vital signs, & documenting patient’s condition, communicating with the rest of the team In addition, the LPN will assist in performing nurse triage duties as needed.       Responsibilities

      • Demonstrates the knowledge and skills necessary to provide care, based on physical, psychosocial, education, safety, and related criteria, appropriate to the age of the patients served in assigned service area
      • Supports organization’s mission by striving for excellence in all aspects of their job with a focus on positive interpersonal relationship with co-workers
      • Monitors/contacts high risk patients and families, either remotely via telephone or during home visits, as needed for emergent patients
      • Monitors quality and appropriateness of services provided
      • Provides individualized counseling to clients and their families to promote continuity of care
      • Coordinates the transitions of patients from hospitals and sub-acute rehabilitation centers to home care
      • Establishes and maintains professional working relationship with all health care related staff, CMS and agency staff
      • Assists in all other office functions when necessary, to include answering patient calls, making referrals, placing orders, assisting with scheduling, etc.
      • Adheres to agency infection control and safety policies
      • Adheres to the organization’s policy in regards to absenteeism and appearance

      Qualifications

      • Licensed Practical Nurse NJ with minimum 2 years’ experience
      • Current and valid BLS/CPR Certification by American Heart Association
      • Excellent verbal and written communication skills
      • Strong computer skills
      • Excellent organizational and interpersonal skills
      • Car and valid driver’s license

      Working Conditions/Physical Demand: Working with clinical equipment and use of phone and computer; Job characteristics: Client and Customer contact. Must be able to lift minimum of fifty pounds (50 lbs.) with heavier lifting on occasion
