Job Description

- Collects and documents data on each assigned patient on an ongoing basis to include but not limited to the normal systems review, behavioral, psychological, developmental, and age-related factors. Specific ages: adolescent, adult, and geriatric.
- Performs basic and/or advanced nursing task, as appropriate for the department/unit.
- Assists with admission, transfer, and discharge of patients
- Assists patients with activities of daily living
- Oxygen Therapy
- Dressing changes
- Monitor and record vital signs (T, P, R, B/P)
- Aseptic and sterile technique
- Insert and removal of urethral catheters
- Witness permits, procedures, and orders
- Receive critical lab values over telephone and relay information to the RN
- Perform Intravenous Therapy (excluding neonates) to include saline lock flushes
- Collect lab specimens if indicated and sent to lab appropriately
- Blood sugar testing/evaluation/glucometer
- Remove superficial sutures, staples, and clips
- Use of computers
- Participates in team conferences, care planning/problem identification.
- Provides and documents patient (significant other) education based on physical, behavioral, psychological, developmental, and age-related factors, after identifying barriers to learning.
- Follows safety practices and helps to maintain safe clean environment.
- Records and reports to the RN all observed abnormal and/or significant findings to include but not limited to symptoms, reactions, responses to treatments, medications, and changes in the patient’s condition.
- Documents all patient care activities and/or observations according to policies and procedures.
- Attends in-service programs, all mandatory required training/education and unit/departmental/service meetings.
- Works with RN’s and other staff to ensure that appropriate stocking of supplies and linen is maintained.
- Works in callbacks as assigned, including taking off calls, completing orders, assisting with form completion, assisting physicians with chart reviews, and returning calls to patients.
- Flexibility in scheduling may be necessary based on patient care needs.
- Maintains performance, patient and employee satisfaction and financial standards as outlined in the performance evaluation.
- Performs compliance requirements as outlined in the Employee Handbook
- Must adhere to the DCH Behavioral Standards including creating positive relationships with patients/families, coworkers, colleagues and with self.
- Performs essential job functions in a manner that ensures the safety of patients, visitors and employees.
- Identifies and reduces unsafe practices that may result in harm to patients, visitors and employees.
- Recognizes and takes appropriate action to reduce risks and hazards to promote safety for patients, visitors and employees.
- Requires use of electronic mail, time and attendance software, learning management software and intranet.
- Must adhere to all DCH Health System policies and procedures.
- All other duties as assigned.
Overview
Under the supervision of a Registered Nurse, the LPN administers direct and indirect patient care according to experience, training, certifications, and demonstrated ability within the limits of this job description.

Qualifications
Current Alabama LPN Licensure. Must be BLS certified upon hire or within 90 days of start date. Must be able to read, write legibly, speak, and comprehend English.

WORK CONTEXT
Ability to tolerate prolonged periods of standing and walking. Exposure to human body fluids. Fast paced physician practice.
PHYSICAL FACTORS
Medium work. Exerting up to 50 pounds of force occasionally, and/or up to 30 pounds of force frequently, and/or up to 10 pounds of force constantly to move objects. Able to push stretcher/wheelchair with average adult patient. Able to do frequent stooping and on occasion crouching, crawling or kneeling. Ability to reach reasonable distances in any direction. Ability to run in emergency situations. Must be able to move fingers in a coordinated manner. Must have ability to feel and perceive temperature, texture, shape and size with fingertips. Must be able to perform the duties with or without reasonable accommodation. hearing and vision must be normal or corrected to within normal range. Color vision normal. Physical presence onsite is essential.
