      Full-Time & Part-Time Opportunities Available

      We are currently seeking Licensed Practical Nurses (LPNs) to join our team in Greenwich (including Old Greenwich, Cos Cob, Riverside), Stamford, New Canaan, Darien, Norwalk, Westport, and Wilton.

      Job Responsibilities:

      • Provide direct patient care and case management in accordance with physician orders

      • Conduct patient admissions and create individualized plans of care

      • Obtain and document medical history from patients and families

      • Perform brief physical assessments including vital signs and mental status

      • Educate patients and families about care plans and services

      • Deliver skilled nursing services in compliance with HIPAA regulations

      • Evaluate and adjust nursing care plans based on patient needs

      • Submit patient documentation in a timely and accurate manner

      • Collaborate with interdisciplinary team to coordinate total patient care

      Qualifications


      Qualifications:

      • Valid LPN license in the state of Connecticut

      • Current TB test (within the last 12 months) and recent physical exam (within 6 months)

      • Current CPR certification

      • Valid driver’s license

      • Must be able to attend a 2-day orientation at our Stamford office
