      POSITION TITLE
      LTC Claims Care Management Specialist

      POSITION LOCATION
      This position is available to Virginia residents as Richmond or Lynchburg, VA Hybrid in-office applicants or remote applicants residing in states/locations under Eastern or Central Standard Time: Alabama, Arkansas, Connecticut, Delaware, Florida, Georgia, Illinois, Indiana, Iowa, Kansas, Kentucky, Louisiana, Maine, Maryland, Massachusetts, Michigan, Minnesota, Mississippi, Missouri, Nebraska, New Hampshire, New Jersey, New York, North Carolina, North Dakota, Ohio, Oklahoma, Pennsylvania, Rhode Island, South Carolina, South Dakota, Tennessee, Texas, Virginia, Washington DC, Vermont, West Virginia or Wisconsin.

      *Hybrid in-office would be required if you reside within 50 miles of our Richmond or Lynchburg, VA office.

      YOUR ROLE
      As an Operations team member, you’ll play a crucial role in delivering world class customer service and capabilities to our policyholders—now and in the future. The LTC Claims Care Management Specialist will be responsible for providing evidence-based, short-term care management services to long-term care insurance policyholders who have a high probability of recovering from a functional or cognitive loss. The LTC Claims Care Management Specialist will focus on claims that have reported recovery or request to withdraw their claim. Research and provision of local/ state/ federal resources to provide advocacy and enhance the overall customer experience.

      What You Will Be Doing

      • You will apply person-centered, strength-based care management practices, both telephonically and in writing, with the insured and their family members. This includes reviewing their functional and cognitive assessments, analyzing the claim, to empower the insured to maintain recovery with wellness interventions.

      • You will be able to comprehensively assess and address the insured’s biological, psychological, and social needs. This includes providing education on common medical conditions using understandable terms and may include coaching caregivers on how to best support the insured.

      • You will be able to anticipate and help the insured to problem solve common barriers to recovery, motivate, and engage the insured. This may include suggesting referrals to specialists, recommending home modifications, researching appropriate equipment, and connecting the insured to resources.

      • You will need to research local/state/federal resources and services for the insured and be comfortable advocating for them if requested.

      • You will work independently and in a team environment, will manage and prioritize a work queue with multiple job responsibilities, and identify process improvement opportunities.

      • You will provide feedback on processes and the care management model to enhance the team’s performance and results.

      What You Bring

      • Bachelor’s degree in a field related to care management (health services, gerontology, mental health, psychology, or social work)

      • Minimum 2 years of case management experience with older adults and their families (hospice, hospital, nursing home, or community placements)

      • Demonstrated experience in making sound business decisions using risk management and quality protocols to drive recovery

      • Professional and personal effectiveness in establishing relationships with customers, care providers, discharge planners, vendors, peers and Claims associates

      • Ability to understand and Interpret Insurance Contracts and Long Term Care Benefits

      • Able to understand and interpret Physician Records and Occupational/Physical Therapist records and understand disease progression and recovery expectations

      • Understanding of single and cumulative medical conditions (particularly those common in the aging process), including their effect on physical/cognitive function, as well as their prognosis and rehabilitative potential

      ADDITIONAL

      The base salary pay range for this role starts at a minimum rate of $55,300 up to the maximum of $102,000. In addition to your base salary, you will also be eligible to participate in an incentive plan. The incentive plan is based on performance and the target earning opportunity is 7% of your base compensation. The final determination on base pay for this position will be based on multiple factors at the time of this job posting including but not limited to geographic location, experience, and qualifications to ensure pay equity within the organization.
