What to Expect (Job Responsibilities):
- Perform telephonic and/or virtual outreach to individuals in specialized programs
- Submit utilization/authorization requests to utilization management
- Consult with primary service coordinator to monitor the PCSP
- Engage with clinical healthcare management and interdisciplinary teams for care coordination
- Manage non-clinical needs of members with chronic illnesses, co-morbidities, and disabilities
What is Required (Qualifications):
- BA/BS degree and a minimum of 2 years of experience working with a social work agency
- Experience working with older adults in care management or provider capacity
- Ability to manage a community and/or facility-based care management case load
- Strong communication and interpersonal skills
- Knowledge of health care-related fields
