What to Expect (Job Responsibilities):
- Develop and coordinate holistic care management activities to ensure quality, cost-effective healthcare outcomes
- Create and monitor personalized service care plans for long-term care members
- Facilitate communication between members, their families, and care provider teams
- Provide resource support to members for various needs, including employment and housing
- Collect and maintain member information to ensure compliance with regulatory standards
What is Required (Qualifications):
- Bachelor’s degree and 1 year of related experience, or equivalent experience
- For Illinois Plan, must meet specific criteria such as RN licensure or relevant degree in human services
- Experience working with diverse populations and individuals with disabilities or chronic conditions
- Ability to conduct comprehensive assessments and develop service plans
- Strong communication and interpersonal skills

- Experience working with vulnerable populations, including children and young adults with disabilities
- Knowledge of case management practices and healthcare services
- Familiarity with community resources and support systems
- Previous roles in healthcare or social services settings
