LVN - Discharge Planner for our Transitional Care Center on the Providence Little Company of Mary Medical Center campus in Torrance, CA. This position is Full-Time and will work 8-hour Day shifts. Our TCC currently provides care for approximately 65 patients.
The Care Management Assistant (CMA) collaborates with the patient/family, the acute multidisciplinary team, and post-acute providers to implement the patients discharge plan. The CMA also proactively escalates risks and barriers to discharge to the Case Manager; documents all discharge planning efforts in the patient's medical record in a timely manner; maintains ongoing communication with the multidisciplinary team and follows all regulatory requirements and ministry polices regarding care coordination/discharge planning.
