Job Description

- Must have an associate degree (A.A.), or equivalent from a two (2) year college.
- Must have at least one year’s experience in hospice, home health or a related field.
- Must have a current State LPN or LVN license.
- Must be able to speak and write effectively.
- Must work well with other disciplines to coordinate patient care.
- Must be able to prepare forms for pre and post payment audits.
- Must have a current State driver's license
- Experience in reviewing nurses’ notes for completeness, compliance with physicians’ orders, authorized services, regulations required
- Experience in reviewing admission packets required
- Experience required in reviewing all POC and re-certification for: accuracy, order updates, frequency updates, goal updates, medication profiles review, format and wording, diagnoses update, C.N.A. care plan
- Conducts one-on-one in-services for documentation problems as needed
- Effective written and verbal communication
- Ability to travel - must have reliable transportation
- Clean background and drug screen
