      Job Summary

      Summary

      Under direction of the Nurse Manager, Clinical Coordinator & Charge Nurse, the Community Residence Counselor (CRC ) provides direct patient care and participates in therapeutic milieu within the scope of defined practice. The Pavilion is an adult, unlocked, residential program treating acute and sub-acute psychiatric patients requiring comprehensive psychiatric and neuropsychiatric consultation, assessment, and treatment. The Pavilion is designed to provide unparalleled diagnostic evaluation with an emphasis on evidence-based care, discretion, flexibility, and confidentiality.

      Essential Functions
      Provides direct patient care in accordance with current treatment plan, under the supervisor of the Nurse Manager, Clinical Coordinator and Charge Nurse.
      -Provides and supervises community activities according to needs.
      -This may include driving & transporting patients to and from activities in hospital vehicles.
      -Remains awake and alert, at all times, while on the job.
      -Assists patients in maintaining and keeping various appointments.
      -Clear, concise, and timely documentation and shift notes.
      -Establishes relationships with patients that are helpful and maintains professional boundaries.
      -Participates in patient rounds, status checks, or teams per program guidelines.
      -Completes assigned patient care tasks per program guidelines and/or daily assignment sheet.
      -Works effectively to manage the therapeutic milieu.
      -Provides patients with direction and supervision as needed.
      -Follows established hospital & program policies, procedures, and protocols.
      -Communicates information regarding patient milieu/therapeutic environment to appropriate staff in a timely manner.
      -Communicates observations related to medication side effects, physical and emotional changes to appropriate medical staff in a timely manner.
      -Encourage patients in their activities of daily living.
      -Assesses sleep patterns and assists patients to develop more appropriate sleep cycles notifying clinical team as necessary.
      -Completes mandatory annual in-service requirements, including but not limited to: CPR and de-escalation.
      -Attends and participates in rounds and case conferences when appropriate.
      -Physical ability for participation in patient activities.
      -Other duties as assigned.

      Education
      High School Diploma or Equivalent required and Bachelor's Degree Related Field of Study preferred

      Licenses and Credentials
      Medication Administration Program Certified [MAP - Massachusetts] - Massachusetts Department of Public Health required

      Experience
      One-year patient care or related experience preferred, preferably in a psychiatric setting 1-2 years preferred

      Knowledge, Skills and Abilities
      - Time Management.
      - Excellent verbal and written communication skills.
      - Ability to work as part of a team.
      - Maintains a commitment to professional growth and competency.
      - All employees must possess basic computer skills to use a variety of electronic or online systems (PRISM) for communication, clinical and administrative purposes.
      - Familiarity with Epic preferred.
