      What to Expect (Job Responsibilities):

      - Act as a liaison for patients, ensuring they receive high-quality care and improved health outcomes
      - Assist at-risk patients through case management and primary care provider referrals
      - Coordinate care and interventions by communicating with primary care physicians
      - Close gaps in healthcare by preparing patient lab result letters and following up on lab results
      - Maintain adequate nursing supplies inventory and fulfill nurse practitioner supply orders
      - Perform administrative tasks such as handling telephone and email inquiries, documenting follow-ups, and updating patient records
      - Support Clinical Integration leadership team with special projects

      What is Required (Qualifications):

      - High School Diploma or GED
      - 1-3 years of clinical experience in family practice, in-home health, primary care, palliative care, or hospice settings
      - Basic understanding of patient care, medical terminology, coding procedures, reference tools, and clinical pharmacology
      - Working knowledge of desktop software applications
      - Ability to lift up to 50 lbs
      - Excellent customer service and communication skills
      - Ability to work independently or as part of a team
      - Strong attention to detail, ability to prioritize tasks and meet deadlines
      - Critical thinking skills and initiative
      - Empathy for working with vulnerable populations
      - Positive working relationships with peers and conflict management skills
      - Graduate of an accredited Certified Medical Assistant program

      How to Stand Out (Preferred Qualifications):

      - More than 1 year of experience as a Certified Medical Assistant
      - Experience with electronic medical record systems, preferably Epic
      - Experience working with underserved populations facing socioeconomic barriers to healthcare
      - Bilingual in Spanish
