What to Expect (Job Responsibilities):
- Engage with patients to navigate their healthcare needs and improve communication with medical practice staff
- Collaborate with clinical teams to manage a targeted population of patients
- Accurately document patients' medical histories in the electronic health record (EHR)
- Implement care plans and assist patients with referrals to specialty care and community-based organizations
- Follow security policies to protect patient information while working remotely
What is Required (Qualifications):
- Certified or Registered Medical Assistant or Certified EMT with 2 or more years of experience
- Bilingual in English and Spanish
- Strong interpersonal skills with a focus on empathy and professionalism
- Proficiency in Microsoft Office and experience with electronic health records
- Reliable transportation and a valid state driver's license for occasional travel

- Experience in patient coordination for individuals with complex medical and social needs
- Familiarity with motivational interviewing and behavioral change management techniques
