      Empowering People’s Independence (EPI), currently has Full time Medical Liaison/ Direct Support Professional position available. This position is responsible for scheduling and coordinating medical, dental, and counseling appointments for the individuals EPI serves.

      Job Purpose:

      Assist individuals with brain injury, intellectual disabilities, and other neurological disabilities in achieving a challenging, stimulating, dignified life by serving as an advocate and liaison for and on behalf of individuals’ rights and benefits.

      Essential Functions:

      • Maintain the health and well-being of the individuals EPI serves
      • Schedule and coordinate all medical appointments for the individuals EPI. serves including acute appointments - transport and accompany people to all appointments
      • Notify circle of supports of all appointments and results of appointments
      • Ensure medical provider documents the results of appointments and recommendations on all appropriate form; send copies of consults to appropriate personnel
      • Alert RN and supervisor of any updates needed to IPOP or PONS
      • Assist in obtaining and maintaining needed adaptive equipment/supplies
      • Report all policy violations to appropriate personnel
      • Promote workplace safety

      Non-Essential Functions: (Hiring Manager's discretion)

      • Other duties as assigned within scope of practice.
      • Participate on various committees as able.

      Education and Experience:

      • High school graduate or equivalent and one-year experience in the human service field
      • Current/valid NYS driver’s license and reliable transportation required.
      • Team player
      • Familiar with the use of email, the internet, and online applications a plus!

      Qualifications:

      • Maintain a valid New York State driver’s license; a vehicle is required to perform the job
      • Become medication certified (if required by Funder) and be trained in all required training within 90 days of being hired
      • Solid verbal and written communication skills and ability to communicate with all persons supported and employees within various levels of the organization.
      • Maintain current certifications with all required trainings
      • Must become van certified
      • Ability to assist in wheelchair transfers

      • Must be able to lift, transfer, and move at least 50 lbs.
      • Must be able to talk, hear, stand for extended periods of time, walk, sit, twist, bend, use hand to finger, stretch with hands and arms
      • Specific vision abilities required by this job include:
        Close vision
        Ability to adjust focus
      • Employee must be able to train in and perform Strategies for Crisis Intervention and (SCIP-R)
      • Perform CPR and first aid

      While performing the duties of this position, the employee travels by automobile and/or Agency Van and will be exposed to changing weather conditions.

      Pay Rate - $20.00/hr-$25.50/hr
