Main Duties:

Support the clinical team by signposting patients to the appropriate healthcare professional or service, working as part of the multidisciplinary team.

Job summary
Care Navigator/Data Administrator Forum Family Practice
We are looking to recruit a friendly, motivated, enthusiastic, and ideally experienced Care Navigator/Data Administrator to join our supportive team.
In this role, youll play a crucial part in helping our patients, both face-to-face and over the phone, with their healthcare needs. Youll be a key part of ensuring patients receive the right help at the right time.
Ideally, youll have previous experience working in a GP practice or within the NHS, and some knowledge of SystmOne though this is not essential, as training will be provided.
If you enjoy variety in your work, have strong IT skills, and pride yourself on delivering excellent customer service, this could be the perfect role for you.
We are looking for someone to work a minimum of 25.5 hours per week (MondayWednesday).

Job responsibilities

b. Answer incoming phone calls, transferring calls or dealing with the callers requests appropriately.
c. Process patient requests for appointments.
d. Initiate contact with and respond to requests from patients, team members and external agencies.
e. Enter read/SNOMED CT code data.
f. Data entry of new and temporary registrations and relevant patient information as required.
g. Input data into the patients healthcare records as necessary.
h. Manage all queries as necessary in an efficient manner.
i. Maintain a clean, tidy, effective working area at all times.
j. Support all clinical staff with general tasks as requested.
k. Undertake all mandatory training and induction programs.
l. Attend a formal appraisal with their manager at least every 12 months. Once a performance/training objective has been set, progress will be reviewed on a regular basis so that new objectives can be agreed.
m. Contribute to public health campaigns (e.g., flu clinics) through advice or direct care
Secondary responsibilities
a. Support administrative staff, providing cover during staff absences.
b. Action incoming emails when necessary
c. Scan patient-related documentation and attach scanned documents to patients healthcare records.
d. Complete opening and closing procedures in accordance with the duty rota
