      Reliant Medical Group, Part of Optum

      • Position: Medical Social Worker
      • Department: Care Management
      • Hours: Full-time; Monday - Friday, 8:00AM - 4:30PM
      • Hybrid: In-office and remote work element

      Primary Responsibilities:

      • Assists patients using a “coaching” method rather than “teaching” to improve their health behavior to attain health-related goals
      • Facilitates patient and caregiver access to care and community resources relevant to patient’s needs
      • Assists patients/families with issues related to insurance, such as lack of coverage (or adequate coverage), medication affordability, and benefits-related issues
      • Provides counseling and psychosocial assessment of adjustment issues, support systems, caregiver issues, and crisis intervention
      • May provide clinical services to patients and families in the form of short-term counseling (<>
      • Works in collaboration with assigned RN partner to address problems and/or concerns about plan of care, as needed, and intervenes regarding adherence issues

      Required Qualifications:

      • Graduate of an accredited school of social work
      • Master’s Degree and active, unrestricted licensure as a LCSW/LICSW in the Commonwealth of Massachusetts
      • CPR Certification or able to obtain within 30 days of employment
      • Current Massachusetts driver’s license
      • Proficient in computer use, the Internet and health information technology
      • Ability to travel frequently to hospitals, skilled nursing facilities, patient’s homes and other sites where patients received care as needed

      Preferred Qualifications:

      • 2+ years of practice in a medical setting
      • Proven excellent communication, interpersonal and organizational skills, with a flexible and creative approach to problem-solving
      • Demonstrates successfully able to work independently as well as a member of an interdisciplinary team
      • Demonstrates possesses good clinical judgment and decision-making skills

      Unless certification, licensure or registration is required, an equivalent combination of education and experience, which provides proficiency in the areas of responsibility listed in this description, may be substituted for the above requirements.

      Physical Requirement:

      • Physical health sufficient to meet the ergonomic standards and demands of the position
