What to Expect (Job Responsibilities):
- Provide care coordination to Medicare beneficiaries receiving LTSS and/or custodial care
- Perform telephonic or face-to-face clinical assessments for the identification, evaluation, coordination, and management of member's needs
- Coordinate care for members with chronic illnesses, co-morbidities, and/or disabilities to ensure efficient utilization of health benefits
- Develop individual care plans and establish short and long-term goals in collaboration with members and healthcare professionals
- Assist with the implementation of member care plans, including authorizations/referrals for utilization of services
What is Required (Qualifications):
- Minimum of 3 years of experience working with individuals with chronic illnesses, co-morbidities, and/or disabilities in a service coordinator, case management, or similar role
- Master's Degree in Social Work required
- Licensed Registered Nurse, Clinical Social Worker (LCSW), or Licensed Master Social Worker (LMSW) required
- Proficient PC skills with Microsoft Office Suite
- Strong knowledge of Medicare/MMP LTSS
How to Stand Out (Preferred Qualifications):
- Field experience (home health, etc.) preferred
- Remote position servicing Wayne and Macomb counties in Michigan. Candidates must be located within Wayne, Macomb, or surrounding counties.
