      Schedule: 12:30pm - 9:00pm, every 3rd weekend as part of the regular rotation.

      General Purpose of Job:

      The Pharmacy Technician, under the supervision of a Registered Pharmacist, performs medication reconciliation for patients in patient care areas as assigned. The Pharmacy Technician must also notify the provider of any discrepancies between the patient’s problem list, medications from home, and medications list in the EMR.

      Essential Duties and Responsibilities:

      • Maintains utmost level of confidentiality at all times.

      • Adheres to hospital policies and procedures (including using two patient identifiers at all times).

      • Demonstrates business practices and personal actions that are ethical and adhere to corporate compliance and integrity guidelines.

      • Understands the legal limitations and scope of practice of a pharmacy technician and refers problems or situations requiring professional judgement to a pharmacist.

      • Conducts an appropriate and respectful interview of the patient, or family member if necessary, to construct an accurate list of home medications including the medication name, dosage form, dose, route, frequency, strength/concentration, indication(s) of use, and time/date the last dose was administered.

      • Records patient-reported problems, medications, adverse reactions and allergies in the EMR.

      • Reviews patient-reported allergies and reactions in pre-existing EMRs.

      • Identifies patient’s primary healthcare provider(s) and pharmacy/pharmacies.

      • Contacts outside healthcare providers and pharmacy/pharmacies when needed to clarify or verify patient-reported lists, and documents communications in the EMR.

      • Identifies patient barriers to education about medications, access to medications and/or compliance.

      • Appropriately notifies the pharmacist/provider or flags medication issues in the EMR.

      • Demonstrates knowledge of the various dosage forms of medications, including concentrations of common commercially available and compounded suspensions and solutions.

      • Demonstrates knowledge of growth and development over the entire life span of a person.

      • Appropriately uses age-specific concepts and language when communicating with employees, visitors, patients and others.

      • Retrieves, enters, saves and prints data, procedures and reports without appreciable assistance.

      • Accomplishes routine and non-routine miscellaneous assignments in accordance with procedures or instructions, and time frames.

      • Correctly re-establishes priorities based on workload and patient needs.

      • Answers phone calls promptly and politely.

      • Serves as a teacher and trainer for new pharmacists and technicians.

      • Routinely checks all available means of communication including e-mail and mailbox.

      • Communicates with co-workers when leaving the work area.

      • Performs other duties as assigned.

      Education and/or Experience:

      • High school diploma or general education degree (GED); current enrollment in an ACPE Accredited Pharmacy School.

      • Completion of a pharmacy technician training program.

      Certificates, Licenses, Registrations.

      • Pharmacy Technicians must be registered with the Texas State Board of Pharmacy and be in good standing.
