What to Expect (Job Responsibilities):
- Serve as the member's liaison throughout the life cycle of the program
- Conduct telephonic visits with members, adhering to standard operating procedures
- Identify and connect members to resources for addressing Social Determinants of Health (SDOH)
- Collaborate on creating action plans for member barriers
- Accurately document all phone calls, interventions, and appointments
What is Required (Qualifications):
- Must have a High School Diploma or GED
- 2+ years of customer service and/or healthcare experience in a fast-paced environment
- Strong working knowledge of medical terminology and healthcare landscape preferred
- Excellent written and verbal communication skills
- Proficiency with Microsoft Office applications, including Word, Excel, and PowerPoint
How to Stand Out (Preferred Qualifications):
- Associate Degree or higher from an accredited college
- Bilingual skills, with a focus on Spanish language proficiency
