At least two (2) years’ experience working with the Geriatric population.
A thorough knowledge of Alzheimer’s Disease and related memory impairments.
Experience with positive management techniques for addressing challenging behaviors related to dementia,
Alzheimer’s training course as required by state regulations.
Bachelor’s or Master’s Degree in Social Services, Human Services, or Social Work or
related areas of study or an active RN license in the state employed; license must be valid and in good standing.

Position Overview:
The Memory Care Facilitator is responsible for the day to day
operations and the provision of quality care to our residents living on our Memory Care Unit. The Memory Care Facilitator alerts the Director of Nursing Services when a resident has a change of condition.
Key Responsibilities:
• Coordinate services from social services, dietary, maintenance, and housekeeping
departments;
• Maintain a clean and attractive environment;
• Ensures state and federal regulations are maintained;
• Ensure staffing levels remain above state regulations;
• Utilize the dementia programs and systems to maintain a secure and safe environment;
• Ensure residents enjoy daily activities that are structured and well organized.
• Perform other tasks as assigned.
