- Provide one-to-one support via face to face or telephone consultation for patients experiencing low to moderate mental health issues such as anxiety, low mood, or stress.
- Offer psychoeducation such as motivational interviewing and solution focused approaches.
- Support/signpost patients to access community mental health services, voluntary sector support, and digital mental health tools.

Job summary
To provide support to patients with mental health and wellbeing needs within a primary care setting, working collaboratively with GPs, nurses, social prescribers and mental health practitioners. The role focuses on early intervention, prevention, and signposting/referring to appropriate services, supporting patients to improve their mental health and overall wellbeing.

About us
Mewstone Primary Care Network has a list size of approximately 32,000 patients. The PCN practices are Church View Surgery, Wembury Surgery, Yealm Medical Practice and Dean Cross Surgery, the practices share the same ethos of supportive learning.
You will work alongside our GPs, nurse teams, administrators and extended workforce including clinical pharmacists, pharmacy technicians, first contact physiotherapist, mental health practitioner, social prescribing link workers, health & wellbeing coaches and care co-ordinators.
All network practices are high achieving practices with a commitment to mentoring and supporting their staff.
